State of Illinois Health Requirements for the 2025-2026 School Year for Kindergarten and 2nd Grade
Kindergarten Students:
● Physical Exam on IL form (Parents please complete the yes/no questions on the back and sign and date)
● Complete immunization record signed by a doctor: check with your healthcare provider for compliance
● Dental Exam on IL form due May 15, 2026(dated within 18 months of May 15th deadline)
● Eye Exam (performed by optometrist) on Illinois form dated within one year of the first day of school.
● Birth Certificate
Second Grade Students:
● Dental Exam on IL form due May 15, 2026 (dated within 18 months of May 15th deadline

We made our Raise Craze goal! Currently, $10,045 has been raised. It closes on Thursday. All monies and Acts of Kindness are due then. Thank you for your support!
