Early Childhood Screening information for the 2026-2027 school year:
State of Illinois Health Requirements for the 2025-2026 School Year for Kindergarten and 2nd Grade:
Kindergarten Students:
● Physical Exam on IL form (Parents please complete the yes/no questions on the back and sign and date)
● Complete immunization record signed by a doctor: check with your healthcare provider for compliance
● Dental Exam on IL form due May 15, 2026(dated within 18 months of May 15th deadline)
● Eye Exam (performed by optometrist) on Illinois form dated within one year of the first day of school.
● Birth Certificate
Second Grade Students:
● Dental Exam on IL form due May 15, 2026 (dated within 18 months of May 15th deadline

KH enjoyed their incentive for the classroom with the most completed acts of kindness per student average today. They discussed the story elements of the movie afterward, which is a part of our a Benchmark literacy curriculum!
