5th grade has started March Picture Book Madness!

State of Illinois Health Requirements for the 2025-2026 School Year
Sixth Grade Students:
Physical Exam (Sports physical NOT acceptable, IL Certificate of Child Health Exam form required) Parents please complete yes/no questions on the back and sign and date
Complete immunization record signed by a doctor, including (1)Tdap,(1)Meningitis-these are 6th grade requirements
Dental Exam on IL form due May 15, 2026(dated within 18 months of May 15th deadline)
