Parent Notification of Health Requirements Deadline
State of Illinois Health Requirements for the 2020-2021 School Year
Students will NOT be able to attend school whether full remote or in-person after October 15, 2020 until ALL required physical exams and immunizations are received.
Students not having these on file or a written statement of the appointment date with a healthcare provider will be excluded from school effective October 16th until the requirements are on file.
Kindergarten Students:
Physical Exam (Parents please complete yes/no questions, sign, and date)
Complete immunization record signed by healthcare provider
Eye Exam or waiver
6th Grade Students:
Physical Exam (Sports physical NOT acceptable-Certificate of Child Health Exam form required) Parents please complete yes/no questions, sign, and date
Complete immunization record signed by healthcare provider- Tdap & meningitis are 6th grade requirements
9th Grade Students:

Chromebook pick up will be Thursday 8/20 from 11am-6pm or Friday 8/21 from 7:30am-3:30pm. A parent or guardian will need to be the one to pick up as an agreement will need to be signed. If you are unable to make one of these times, please call the office to arrange a pick up time. Chromebooks need picked up at each student's attendance center. Login information will be given at this time.
All students 9th through 12th grade will receive a chromebook for use during the blended and full remote learning options. Our chromebook pick up dates and times will be: Thursday, 8/20 from 11-6 pm and Friday, 8/21 from 7:30-3:30 pm at the High School.
