State of Illinois Health Requirements for the 2025-2026 School Year for Kindergarten and 2nd Grade:
Kindergarten Students:
● Physical Exam on IL form (Parents please complete the yes/no questions on the back and sign and date)
● Complete immunization record signed by a doctor: check with your healthcare provider for compliance
● Dental Exam on IL form due May 15, 2026(dated within 18 months of May 15th deadline)
● Eye Exam (performed by optometrist) on Illinois form dated within one year of the first day of school.
● Birth Certificate
Second Grade Students:
● Dental Exam on IL form due May 15, 2026 (dated within 18 months of May 15th deadline

Yearbook for those that purchased them will be going home today. They will have the student's name and teacher in the bottom left corner. There are no additional yearbooks available to purchase.

Mrs Martens' 3rd Grade learned about George Hutson, a local artist from right here in Warren County known for his dinosaur paintings! The students enjoyed an original painting from his exhibit at the Buchanan Center For the Arts, "Mezozoic Tectonics" - looking closely for brush strokes and paint texture.
