All Illinois children in 6th grade are required by the Illinois State Board of Education to have a dental examination on file by May 15. 2022. School dental examinations must have been completed within 18 months prior to the May 15th deadline.
A Dental Examination form or a Dental Examination Waiver form must be submitted to the school by May 15, 2022. Please contact the school office with questions. Thank you!
